CHC dos Caetés promotes educational action on patient safety and reinforces the culture of incident reporting
Training brought together more than 600 professionals and achieved 91% adherence; initiative aims to strengthen safety and quality in hospital care

As a reference in medium and high complexity in northeastern Pará, the Regional Public Hospital of Caetés (HRPC), in Capanema, held an educational action aimed at the unit's professionals from June 16 to 20, focusing on reducing incidents related to patient safety. The activity was conducted by the Patient Quality and Safety Center (NQSP) and had the theme "A pillar of safety for the patient."
The program was held in the hospital's auditorium, with schedules in both day and night shifts, in order to accommodate all work teams. The goal was to raise awareness among employees about the importance of incident reporting as an essential tool in identifying risks and promoting a safer and more efficient care environment.

The training was conducted by the coordinator of the NQSP, Talita Serra, and by the nurse of the center, Suzanne Carvalho. For Talita, the event represented a milestone in consolidating the culture of patient safety within the unit. "The positive feedback from participants reinforces that we are on the right track. Transparency, responsibility, and continuous learning are the pillars that support our commitment to safer care," she emphasized.
Suzanne Carvalho highlighted that incident reporting is essential for improving processes and strengthening safety. "This record allows us to recognize opportunities for improvement, reduce failures, and ensure more qualified care for both patients and professionals," she stated.

User Ana Luiza Sousa da Costa, 18 years old, a resident of the municipality of Primavera, was at HRPC for nasal surgery and highlighted the importance of the initiative. "I felt very calm realizing that the hospital cares about these details. When I arrived, they quickly corrected a mistake in my mother's name, and that gave me security. They also explained to me that the bed rails should always be raised, and I found that very important," she reported.

Results - The action had the participation of 606 employees and third parties, registering an adherence rate of 91%. From January to June of this year alone, HRPC has already recorded 2,122 notifications, reaffirming the unit's commitment to the culture of patient safety.
Care - In the first five months of 2025, HRPC performed more than 176,000 services, including consultations, surgeries, and hospitalizations. The average satisfaction rate of users reached 98.68%, a figure obtained from continuous evaluations conducted with patients and companions.
